Overview:

The MDS Coordinator (RN) is a key member of the interdisciplinary health care team responsible for coordinating the development, completion, accuracy, and transmission of resident assessments and person-centered care plans. This position ensures documentation and coding comply with federal and state regulations, support appropriate reimbursement, and reflect each resident’s needs, preferences, goals, and clinical status. The MDS Coordinator serves as a resource to residents, families, and team members throughout the assessment and care-planning process.

Otterbein Marblehead is situated on 47 acres of land two blocks from the shores of Lake Erie, and just outside the gates of Lakeside Chautauqua - in the heart of Ohio's northern shores and islands vacation destinations. This senior care community features 57 independent living patio homes and apartments, 37 assisted living care units, and 20 “5-Star” nursing care suites (in two innovative “GreenHouse” homes).

Responsibilities:

• Coordinate and complete accurate, timely MDS assessments in accordance with federal and state requirements.
• Create, maintain, and update individualized, person-centered care plans, including rehabilitative programs, in collaboration with the interdisciplinary team.
• Review medical records and obtain information through resident, family, and staff interviews to support accurate MDS coding and care planning.
• Ensure the medical record contains documentation that supports assessment findings, coding, services, and reimbursement.
• Coordinate assessment data entry, submission, and transmission within required timeframes.
• Monitor and guide contributions from nursing and other disciplines to ensure care plans and assessments are complete, consistent, and accurate.
• Represent nursing in care conferences and communicate with residents and families regarding care needs, goals, and plans.
• Review and manage reimbursement processes in accordance with applicable rules and procedures.
• Identify changes in resident condition and recommend appropriate updates to care, treatment, or medication for consideration by the responsible clinical provider.
• Participate in quality assurance and performance improvement activities and nursing department meetings.
• Remain current on long-term care regulations, clinical standards, MDS requirements, and reimbursement practices through continuing education, workshops, and professional literature.
• Provide staff education, in-services, and one-on-one training as needs are identified.
• Follow safety, infection prevention, privacy, confidentiality, and organizational policies and procedures.
• Promote resident autonomy, dignity, individuality, and informed decision-making whenever possible and appropriate.
• Communicate professionally and respectfully with residents, families, visitors, and team members.
• Make prompt, accurate judgments regarding resident care and emergencies within the scope of the role.
• Must be willing to take an on-call rotation
• Willingness to provide floor coverage support when necessary
• Perform other duties as assigned.

Qualifications:

• Education: Graduate of approved nursing program - RN
• Licensure: Current, unrestricted Ohio Registered Nurse license and good standing with the Ohio Board of Nursing required.
• Experience: At least three years of MDS experience in a long-term care or nursing home environment preferred; post-acute and geriatric nursing experience preferred.
